***Something different***
Not all surgeries are those that one wants to perform, but they must be done.... Most of the times we try to rescue and preserve a limb, it is essential that every living being performs its vital functions to the maximum with their hands and feet intact, the skill of our movements coordinated with our thoughts makes us different from the vast majority of species, most of the time we do not consider it so until something outside the ***"everyday"*** happens. That's how the story of our patient named Aida began.

No todas las cirugías son aquellas que uno quiere realizar, pero hay que hacerlas… La mayoría de las veces intentamos rescatar y preservar una extremidad, es indispensable que todo ser vivo desempeñe al máximo sus funciones vitales con sus manos y pies intactos, la destreza de nuestros movimientos coordinada con nuestros pensamientos nos hace diferentes de la gran mayoría de las especies, la mayoría de las veces no lo consideramos así hasta que pasa algo fuera de lo ***“cotidiano”***. Rays are "cartilaginous fish", along with sharks and manta rays, which means that their gills are protected by extensions of skin called gill slits. These fins in turn are supported by arches made of cartilage. And a key feature of the arches are appendages called gill rays that fan out, similar to how fingers fan out. The human limbs (upper and lower limbs) represent one of the most complex mechanisms developed since their evolution. More than a century ago, scientists dismissed a proposed theory about how human limbs evolved from gills, given the lack of evidence in the fossil record. That theory, however, is being revised in the light of new genetic results recently published in the journal Development. ***An accident***
One morning like any other, not so different or so different, Aida, a 46-year-old woman left her house for work, usually her eldest son offered her a ride on his motorcycle, so after rushing with breakfast she went immediately to meet him. The weather was sunny one morning with a little more and more traffic, not abnormal for a city full of hard-working people.
At the bus stop in front of the hospital, a large number of passengers get off that morning to go to their doctors' offices, children, adults and the elderly parade down the aisles until they each take their seats in front of their specialists' offices. I was seeing one of my regular patients named Maria, who was coming in for a sprained right ankle when one of the residents in charge of the emergency suddenly called me. Apparently it was very serious so I had to leave Mrs. Maria behind, apologize and run to the emergency room.
While a large medical team saw the patient, a doctor explained the case to me, because it was about an adult woman who, after a collision with a bus, had been run over by the monstrous vehicle that was recklessly going at a very high speed. The patient's name is Aida.

It was not an easy scene to assimilate, since when she was run over by such a heavy vehicle the lower right limb of her was practically shattered, her bones very badly fractured at the femur level, and a large lesion at the skin level of her of great magnitude.

***Un accidente***
Una mañana como cualquier otra, ni muy diferente ni tan distinta, Aida, una mujer de 46 años sale de su casa con dirección al trabajo, normalmente su hijo mayor le ofrecía llevarle siempre en su moto por lo que luego de apresurarse un poco con el desayuno fue inmediatamente hasta encontrarse con él. El clima asomaba una mañana soleada con un poco de tráfico cada vez más agitado, nada anormal para una ciudad llena de gente trabajadora.
En la parada del autobús que se ubicaba frente al hospital una gran cantidad de pasajeros se baja esa mañana para ir a las consultas de sus médicos, niños, adultos y ancianos desfilan los pasillos hasta que cada uno toma asiento frente a los consultorios de sus especialistas. Estaba viendo a una de mis pacientes habituales de nombre María, quien acudía por un esguince en el tobillo derecho cuando de repente me llamó uno de los residentes encargados de la emergencia. Al parecer era muy grave por lo que tuve que dejar atrás a la Sra. María, disculparme e irme corriendo a la sala de emergencias.
Mientras un gran equipo médico veía a la paciente un médico me explicaba del caso, pues se trataba de una mujer adulta que luego de una colisión con un autobús, ella había sido arrollada por el monstruoso vehículo que imprudentemente iba a una velocidad muy alta. El nombre de la paciente: Aida.
No era una escena fácil de asimilar, puesto que al ser arrollada por tan pesado vehículo el miembro inferior derecho estaba prácticamente destrozado, los huesos muy fracturados a nivel del fémur, y una gran lesión a nivel de la piel de gran magnitud.

***Surgery room***
We had to take her immediately to the operating room, the severe injuries needed immediate repair if possible, but there was a problem.... The blood was not reaching her toes, so worryingly we suspected a thigh injury, the examination by the vascular surgeon was extensive and verified what we feared from the beginning.
The solutions to these facts are conclusive, they have to be taken quickly and, above all, the patient's life has to be weighed up, so after informing the relatives of the seriousness of the situation, we proceeded to perform the procedure of "complete amputation of the lower limb".
Because of the location of the injury it was necessary to act quickly and stop the bleeding, every minute that was lost represented blood being lost and less chance of survival. It was thanks to the action of multiple specialists that the procedure was performed quickly and efficiently. A surgery of more than 6 hours and a patient in delicate health conditions.
It is not easy to wake up after an accident, in a hospital with an amputation of the entire lower limb, especially when that same morning everything seemed normal. However, Aida's new story was just beginning, with the support of her family, friends and all the health staff, and she was able to gradually regain the courage to attend the check-up consultations again.
With the great results of the healing process, we hope that in the not too distant future our dear friend will be able to use an "exoprosthetic", they are highly expensive in any country, but surely with the initiative of the people around her she will be able to achieve it.

***Quirófano***
Tuvimos que trasladarla a ella de inmediato al quirófano, las lesiones tan severas ameritaban de una reparación inmediata de ser posible, pero había un problema… La sangre no le llegaba a los dedos de los pies, por lo que preocupantemente nos sospechamos una lesión arterial a nivel del muslo, la exploración a cargo del cirujano vascular fue extensiva y verificó lo que desde el principio temimos.
Las soluciones ante estos hechos son contundentes, se tienen que tomar rápido y se debe por sobre todo sopesar la vida del paciente, así que sin que luego de informarle a los familiares de lo grave de la situación procedimos a realizar el procedimiento de “amputación completa del miembro inferior”.
Por la ubicación de la lesión era necesario actuar rápido y detener el sangrado, cada minuto que se perdía representaba sangre perdiéndose y menos posibilidades de sobrevivir. Fue gracias a la actuación de múltiples especialistas que se realizó el procedimiento de manera rápida y eficaz. Una cirugía de más de 6 horas y una paciente en delicadas condiciones de salud.
No es fácil despertar luego de un accidente, en un hospital con una amputación de todo el miembro inferior, sobre todo cuando esa misma mañana todo parecía normal. No obstante, la nueva historia de Aida apenas comenzaba, con el apoyo de su familia, sus amigos y todo el personal de salud, poco a poco ella logró poder recuperar los ánimos para asistir nuevamente a las consultas controles.
Con los grandes resultados de la cicatrización esperamos que en algún futuro no muy lejano nuestra querida amiga pueda usar una “exoprotesis”, son altamente costosas en cualquier país pero seguro que con la iniciativa de las personas que tiene alrededor lo podrá lograr.
