Last night one Omani elderly lady came with multiple problems. She was having severe pain in around her right hip joint and surrounding are particularly at the thigh. The pain also radiates to the lower legs. This was her long-standing problem but recently increased in severity. 

She had a second and new-onset complaint of painful swelling of her right great toe. When I saw her toe, I found that it was infected with a collection of pus on nail fold. It was paronychia.

Paronychia:

It is a skin and soft tissue infection of the nail fold and sometimes nails bed also. Means, usually the underlying bones are unaffected but if the condition turns into a chronic, long-standing problem, it can be associated with a bone infection which is known as osteomyelitis. 

It is often found in patients who have the habit of nail-biting (in case of finger paronychia). Any foreign body penetrating wound, needle stick injury may lead to paronychia. 

Treatment options include incision and drainage of the wound, i.e. minor operation, and antibiotics. If the infection involves the nail bed, the nail needs to be removed.
