We will take anything that helps target specific mutations in many types of cancer, particularly breast cancer. Trials at the molecular level that go beyond preclinical predictions are the holy grail. Gotta read the study in the night although is a shame the response of Neratinib is not superior to current therapies.

Treatments may include surgery, chemo and radio therapy as well. Treatments depend on stages of cancer.
Complete removal of the cancer without damage to the rest of the body (that is, achieving cure with near-zero adverse effects) is the ideal goal of treatment and is often the goal in practice. Sometimes this can be accomplished by surgery, but the propensity of cancers to invade adjacent tissue or to spread to distant sites by microscopic metastasis often limits its effectiveness; and chemotherapy and radiotherapy can have a negative effect on normal cells.[2] Therefore, cure with nonnegligible adverse effects may be accepted as a practical goal in some cases; and besides curative intent, practical goals of therapy can also include (1) suppressing the cancer to a subclinical state and maintaining that state for years of good quality of life (that is, treating the cancer as a chronic disease), and (2) palliative care without curative intent (for advanced-stage metastatic cancers).

Enlightened information about this kinases as a cell in human body which it going haywire will cause cancer. And true the only way to eradicate cancer that affects some organs is cutting off but i suppose the type of cancer that may be damaging which i know is prostrate cancer and men face this at late age of their life even when they are still fertile and some doctors to asked for sperm preservation for those group of men because they wont be able to ejaculate. It's always pleasant to read about this type of researchers, because it speaks of a favorable future. We are still in "diapers" when it comes about genes, enzymes and other molecular structures, there are still so many of them to discover and some others that we need to actually understand what they do.  But the possiblity of having medicaments personalized even for each genetic sequence will change the life of tons of patients.

This could take years and years, maybe we won't be able to see the results of these discoveries by ourselves, but the generations to come will have unique opportunities to them. Especially with cancer, as you put it here, but also with a lot of incurable diseases nowdays.

It's really good information. If Neratinib effects HER2 and HER3, it means doctors can use it for breast cancer, right? So what are the advantages of Neratinib if we compare with other therapies such as Trastuzumab and what are the side effects of it? Doctors mostly scared of heart effects of this kind of therapies. I hope science world can find a solution for it.

If only the cure could be found I'd still have my awesome uncle who didn't survive chronic myeloid leukemia. Always happy to hear a breakthrough in this fight against cancer as I'm sure many have been victims.

Research never end in combating cancer.Am glad that  group of erudite scholars  are not resting on their oas.Its the over expression of these kinase that are most time responsible for the development of cancers.They are primers for the starting up of DNA replication but things may turn really bad and so good cells turn to bad cells.Discovery of the inhibitor of these kinase should not stop.

The fight against cancer has been age-long, but I believe strongly that this battle is about to be won finally. As much as biology is trying; technology on the other other is complimenting the efforts. And among other things, I have my eyes on nanobots; and I believe that one day, it could provide a knockout blow to cancer. I have hope in the future.

Thanks for  sharing this interesting research news..just read the  full paper. I found that the efficacy of  neratinib  is not that great in comparison to other approved therapies (targeting oncogenic alterations in EGFR, ALK, ROS1 and BRAF ) and may not work as a good monotherapy to treat wide variety of HER2- and HER3-mutant cancers.  The well-know problem with neratinib is it induces serious diarrhea in patients. Thats why they are required to give mandatory anti-diarrhoeal prophylaxis with neratinib. But hey every drugs have their good and bad sides in terms of side effects and efficacy. 

Neratinib was recently approved by FDA to prevent recurrence of early-stage HER2-positive breast cancer. Neratinib HER Mutation Basket Study (SUMMIT) looks like a very promising phase II clinical trial to watch closely for and its future strategy to go into Phase III trial. There is certainly a promise for this work to move forward if novel combination of neratinib with other less toxic anti-cancer drug is used. Using neratinib as a monotherapy would be a hard sell. Just my opinion after looking at the data and following the discussion of authors.  Cheers to science!

Great post.  Perhaps derivatives of neratinib will end up being effective treatments for this specific type of cancer.

I previously worked with erlotinib, a chemotherapeutic EGFR (HER1) inhibitor. Of course genetic variance in tumors is very important for the response to a drug - that's why this work is exceptional.

But from what I understood, a main problem of RTK inhibitors is the quite fast development of tumor resistance.

Too-lazy-to-use-pubmed-question: Do you have any idea if the aquiration of resistance was yet connected to certain genes or mutations?
