That's because of inconsistencies in the returned values of the account history (Steem node related). I reported this recently <a href="https://github.com/steemit/steem/issues/3116">here on GitHub</a> and I hope they will finally fix that. It's a known bug since Oct 27, 2018 and in my eyes it's a major one, because exchanges might have problems with uniquely identifying single operations within one transaction.
